RE: Discussion – Week 1
COLLAPSE
I choose to look into the Association of Women’s Health, Obstetrics, and Neonatal nurses’ website. According to AWHONN (2020), the mission statement describes a desire to help support and empower nurses caring for women, newborns, and their families through education and research. Research conducted by AWHONN includes integrating behavioral and biological science for clinical practice to improve quality patient care. (AWHONN, (2020). As explained by AWHONN (2020), they believe that research builds a foundation of using evidence to improve patient outcomes and is an interdisciplinary effort. Some analysis priorities listed on the site include support to reduce maternal mortality, evidence-based practice training, and improved patient outcomes. (AWHONN, (2020).
I believe that the Association of Women’s Health, Obstetrics, & Neonatal nurses are grounded in evidence-based practice. One reason is that the organization has a Journal of collected articles called the Journal of Obstetrics, Gynecology, and Neonatology Research Review. (AWHONN, (2020). The website also mentions the presence of an evidenced-based practice training program, receiving research grants since 1993, and the commitment expressed of incorporating evidence-based practice as a foundation to improve care in Obstetrics. (AWHONN, (2020). An example of interdisciplinary partnering I found on the website is a consensus statement talking about the Emergency Nurse’s Association collaborating with AWHONN to prevent emergencies related to pregnant patients (AWHONN, (2020). The partnering is, I believe, an example of grounding in evidence-based practice.
I do believe that information from AWHONN.org has changed my perception of the website. Before having some background education in nursing research foundations and starting this class, I would not have known to look into evidence-based practice. In the past, when looking onto this website, honestly, I would mainly scroll down the first page, only looking at highlights. Checking for current nursing research and evidence-based practice would not have been something I did. I now feel I have a better understanding of the association and its work to contribute to evidence-based practice and nurse research. I will use this website as a source in the future when needed to help promote my area of practice in Labor and Delivery.

As nurses, we need to stay current on the new guidelines of practice to promote the best patient’s outcome (Laureate Education, 2018). I have been introduced to The Agency for Healthcare Research and Quality’s (AHRQ) in undergraduate school as one of research reference source. The AHRQ is an organization which grounded with Evidence Base Practice research to develops the knowledge, tools, and data needed to improve the health care system and help Americans, health care professionals, and policymakers make informed health decisions. This organization is a lead federal agency that is in charge of improving of the safety and quality of American healthcare system (Agency for Healthcare Research and Quality, 2014a).
Its mission statement appears in the About section which states: [The Agency for Healthcare Research and Quality’s (AHRQ) mission is to produce evidence to make health care safer, higher quality, more accessible, equitable, and affordable, and to work within the U.S. Department of Health and Human Services and with other partners to make sure that the evidence is understood and used] (Agency for Healthcare Research and Quality, 2014b).
In the About section, the AHRQ highlights their goal is to improve the quality of lives of the patients. In order to do that, the AHRQ provides the three core competencies to help in guiding healthcare professionals and other healthcare systems in their practice. These competencies focus on healthcare system research, practice improvement, and data and analytics.
The AHRQ is a well-grounded EBP because every topic on the website is backed by EBP and it is also lead government research site. Based on the information that I found in the website, I still trust AHRQ for my EBP research. The AHRQ has the quadruple aims which are: improve population health, enhance patient experience, lower health care cost and improve work life of health care workers (Melnyk & Fineout-Overholt, 2018). For examples, the AHRQ provide tools for healthcare professionals to make clinical decisions, budget planning to help government reduce healthcare cost, and data and analytics research for best practice guidelines. By implementing these measures, patients receive a higher quality of care and healthcare professionals are satisfied with their career and work life balance.

Review of Current Healthcare Issues
Throughout my 25-year nursing career, I’ve paid attention to the shortage of nursing staff around me and across the country. The demand for nurses continues to outweigh availability. The shortage seems to be rapidly approaching crisis level, especially during the current unending COVID-19 pandemic. This added stress on many hospitals and specifically on nurses is further expanding the shortage. According to Spurlock (2020), “a projected shortage of 1.1 million RNs in 2022” is expected in the United States. That date is rapidly approaching without concrete solutions to overcome that inevitable predicament for all of healthcare. I wonder, “What is it going to take to shift the balance to have staffing that nurses view as adequate? What can our leaders do to resolve the growing nursing shortage issue?” This discussion reviews a few of the responses or lack thereof to the issue by my healthcare system.
I work in an emergency department where I have far too often felt the crunch of inadequate staffing. The shortage puts a strain on our nurses, the patients, and the system’s goals. According to Haddad and Toney-Butler (2020, Issues of concern section), “Nursing shortages lead to errors, higher morbidity, and mortality rates.” It is just not safe! So, how have the hospital leaders dealt with the shortage of nurses? Well, many institutions like mine have relied on what I call “failed strategic planning” to meet its goals, mission, and vision.
Currently, hospital leaders are seeking to overcome their nursing shortage with bonus programs and outside supplemental staffing. Many hospitals are paying nurses extra hourly pay as an incentive to work extra shifts. Many are relying on agency nurses to temporarily fill larger gaps in scheduling. Those quick fixes come with a hefty price tag and a slap in the face. Some agency nurses are being well-compensated to upwards of $4,000 per week or more – more than double the national average during the coronavirus pandemic (PR Newswire, 2020). Many nurses like myself feel as if this is a slap in the face due to not rewarding the staff that have not quit their jobs to take up the lucrative travel assignments. Where is the reward to retain and build your staff permanently? The crisis needs long-term solutions instead of these temporary band-aids to the nursing shortage’s continuous surge across the country.
In my opinion, many nurses follow the money. The leaders must respond with salary increases for all nurses to promote and motivate retention. The COVID crisis has spotlighted nursing and healthcare, where leaders must shift their strategies to persuade younger people that nursing is still an all worthwhile and rewarding career path to take. In line with Fisher et al. (2020, para. 2), “Due to a national nursing shortage, the active promotion of nursing careers among young people is critical.” The healthcare industry has much to consider including prior failed attempts, the rising competition in technology and other career paths, and the impact a massive shortage of nurses would have on the entire community when deciding how to overcome this critical issue.

RE: Discussion – Week 1
COLLAPSE
Initial Discussion Post – wk 1
The national healthcare issue/stressor that I have chosen is the current nursing shortage. This healthcare issue is one that many of us are experiencing daily. I no longer work for a large hospital, instead I am a nurse manager for a nationwide residential eating disorder facility and we too are feeling effects of a nursing shortage. According to the American Association of Colleges of Nursing, the U.S. is projected to experience a shortage of Registered Nurses (RNs) that is expected to intensify as Baby Boomers age and the need for health care grows. And the fact that nursing schools across the country are struggling to expand capacity to meet the rising demand for care given is compounding the problem. This healthcare issue is impacting my work setting tremendously right now. I am in Arizona at a new facility that we acquired. It is more of an inpatient setting, rather than the residential setting that I am used too. Quite a few nurses left the facility when we took over due to lack of nursing leadership from previous owners and they were not willing to “stick it out” and see if things would improve. On top of the that, the Director of Nursing resigned just last week effective immediately because she could not take the pressure of the staffing crisis. We have brought other nurse managers from other sites across the country to help fill in shifts during this stressful time. We have stepped up our recruiting strategy, increased pay for nurses in this area and are utilizing agency nurses when available.
Registered Nursing is listed among the top occupations in terms of job growth through 2029. The RN workforce is expected to grow from 3 million in 2019 to 3.3 million in 2029, an increase of 7% (Bureau of Labor Statistics, 2020). The shortage of RNs is projected to spread across the country between 201t6 and 2030. In this state-by-state analysis performed by the American Journal of Medical Quality, the forecast is that the RN shortage will be most intense in the South and the West.
