Clinical for this week has begun with the same level of enthusiasm and curiosity. The opportunity that has been given to interact with a wide variety of different patients in primary care settings has tremendously been positive. The clinic receives this week patients who required lab results, prescription refills, follow-up appointment and annual physical. Most of the care has been delivered through telehealth. However, some patients were scheduled to walk into the clinic to receive the care they need.
An 81-year -old male comes to the clinic for an annual physical. BP 138/78, HR 65, RR 16, Weight 239 pounds, BMI 28.3. Patient was diagnosed years ago with diabetes type 2, which is controlled with metformin. Denies any pain. Past medical history includes depression, essential hypertension, hyperlipidemia. Past medical surgery includes appendectomy and tonsillectomy. Family history is insignificant. Patient denies alcohol. Patient is a former patient. Current medications are Lisinopril 10 mg tab, Celera 40 mg tab, Crestor 40 mg and vitamin D 2000 unit.
According to Hollier (2018) more than 30 million people have type 2 diabetes mellitus in the country, which is about 9.4 % of United States population (p. 216). People with a BMI greater than 25.kg/m2 is at risk for type 2 diabetes mellitus. The encounter for general adult medical exam without abnormalities (Z00.00) is one of the diagnosis. Patient has been diagnosed with type 2 diabetes mellitus. Some differential diagnoses include diabetes mellitus type I, Acromegaly, and Cushing’ syndrome (Hollier, 2018, p.217). Patient has also a diagnosis of mixed hyperlipidemia (E78.2) A complete blood count with a panel of lipid are ordered. The fact that the patient has been prescribed some medications. A blood work would be essential in continuous care for this patient.
The management of this patient with multiple diagnoses requires some aggressive treatment. The nonpharmacologic therapy plays an important role in taking good care. Encourage to stay. Ferri (2020) noted that weight loss and the implementation of the Dietary Approaches to Stop Hypertension (DASH) eating regimen, which includes a high diet in fruits, vegetables, and low intake of fat dairy products with decreased animal protein consumption but increased amount of plant protein intake from legumes and nuts should be encouraged (p.744). Patient advised to keep track of blood pressure readings. Exercise on a daily basis as tolerated was discussed also. The avoidance of stress is encouraged for the benefits of a health life. A change in dietary intake and lifestyle modifications are essentially crucial in the management of the patient current condition. All of the above lines were used as a reminder to the patient to keep on practicing the same strict dietary regimen. Patient was told to continue the medication.
Every clinical day has its own challenge and merit. The challenge is nothing but an opportunity to acquire more learning experience. Healthcare field is a lifelong learning. The promise of better days for patients and health professionals cannot be achieved without acquiring great skills and competence that each day has offered.
